How to enroll a patient in the RocheDiabetes RemoteCare solution

RocheDiabetes RemoteCare is an optional module that helps you monitor your patients remotely and may help improve communication channels between healthcare professionals and patients.

To access the features within the solution, you first need to enroll your patient in the Remote Patient Monitoring (RPM) program, if available in your country and center. RPM is designed to provide a defined period of intensive management to patients who may be having difficulties, who are newly diagnosed, or who have recently changed therapy.   • Enroll a patient to RPM

    You can enroll a new patient or an existing one.

    To enroll a new patient:

    1. Click Create new patient from the top-right section of Patient management.
    2. Select the RPM Program checkbox.
    3. Click Next, and complete the information on the screens that follow.
    4. Click Save.
       

    To enroll an existing patient, you can do it either:

    • from Patient management:
      1. Click the arrow at the end of the patient info row for the patient you wish to enroll.
      2. Click Enroll patient.
      3. Select the RPM Program checkbox.
      4. Click Save.
         
    • from Patient view:
      1. Select the patient you wish to enroll.
      2. Click the arrow next to the patient name to access the Patient settings menu, and select Edit patient.
      3. Select the RPM Program checkbox.
      4. Click Save.
         
  • Change the default program settings

    Program settings for RPM are pre-established as defaults. However, users with an HCP master professional role can easily modify and set these settings according to their preferences based on the patient’s needs:

    • Name and duration of the program
    • Therapy adherence rules
    • Notifications regarding deviations from pre-set pattern configurations
       

    All patients enrolled in the program by the center will have the same settings. To modify program settings:

    1. From the actions menu, select RPM Program settings.
    2. Enter or select appropriate values.
    3. Click Save.
