How to create a patient account

Issue

I want to create a new patient in the RocheDiabetes Care Platform.

Solution

To create a new patient:

1. Sign in to your RocheDiabetes Care Platform account.


2. From the navigation bar, click the actions menu.


3. Click Create new patient.
    
4. Select a profile type for the patient:
   • Basic: Diabetes management only.
   • Pick up at center: Patients can pick up test strips at the healthcare office.
   • Home delivery: Enables strip delivery to the patient’s home.
    
   ×Note: Patient profile options vary by region.


5. Use the checkboxes to enable optional features for data sharing and click Next:
   • Patient portal access: Allows patients to remotely upload data from their device, such as blood glucose meters and insulin pumps. It is recommended to leave this selected.
   • Open connectivity: Enables patients to upload data from other supported devices (Windows only).
   • mySugr® data sharing: Allows patients to share their diabetes data using the mySugr® mobile app.


6. Enter your patient's details and click Next.
   ×Note: If using Home delivery, provide the patient’s home address.


7. Complete the patient’s health information and click Next to finish adding the patient.


8. The system confirms that the patient has been added.
